Some students are required to submit a Wage & Tax Statement (W-2). Please confirm on your Financial Aid Document Checklist if you need to submit this document.

Don't have a copy of your W2? How to obtain a copy of your W2:

Go to IRS website
Choose to "Get Transcript ONLINE" (immediately)
Follow steps on the IRS website to complete the request
Select Wage & Income Transcript for the appropriate year

Form 4506-T, to access form click on 4506-T
Be sure to select OPTION 8 on the 4506-T form
You can MAIL or FAX form. Instructions on 2nd page of 4506-T form
