Modesto Junior College will disclose information from a student's education records only with the written consent (see form below) of the student, except:
Student must complete a Release of Student Information Form in the Enrollment Services Office. Proper identification of both the student and parent/guardian in which information is being released to is required. Releases expire exactly one year from the date signed and may be rescinded by the student at any time.
MJC assures that student records are maintained in compliance with applicable federal and state laws relating to the privacy of student records. No district representative shall release the contents of a student record to any third party without the written consent of the student, other than directory information as defined below and information sought pursuant to a court order or lawfully issued subpoena.
Board policy 5040 defines Directory Information as:
If you would like your information blocked so Modesto Junior College cannot release even directory information, you must complete a Notice to Deny Disclosure of Directory Information form. This form must be submitted in-person with photo identification to Enrollment Services, Student Services Building, Room 102 or West Campus, Yosemite Hall, Room 133A. Please note: In addition to not releasing directory information to any person or company, the college may not print your name (without your written consent) in theatrical, musical, or athletic programs, in the commencement program, or as part of the Dean's List that recognizes students for scholastic achievement.
