Appeal Forms

Financial Aid Appeal – You may submit a Financial Aid Appeal if the following criteria applies to you:

You have not met the Financial Aid Satisfactory Academic Progress Standards of maintaining a cumulative GPA of at least 2.0 and/or successfully completing at least 67% of all units attempted.

OR

You have not met your educational goal and have exceeded the maximum time frame (90 attempted units) or have received a degree.

NOTE: You must complete the student portion of the form and take it with your to your counseling appointment.

FAFSA on the Web

FAFSA on the Web Worksheet - Some students prefer to complete something on paper before completing the online FAFSA. A worksheet has been designed specifically for this purpose. The FAFSA on the Web Worksheet tells you what documents (for example, your tax forms) you'll need to refer to as you fill out the FAFSA, and it shows you the FAFSA questions so you can fill in your answers in preparation for applying at www.fafsa.gov. The Worksheet is an optional tool; you can go straight to the FAFSA website to apply if you prefer.

Board of Governors Waiver (BOGW)

Board of Governors Fee Waiver - To receive a BOG fee waiver, students must complete a Free Application for Federal Student Aid (FAFSA) online (www.fafsa.gov) or complete the BOG fee waiver paper application.
