Do you need to make a change to your personal information?  Complete one of the two options below to change your name, address, phone number, or to correct your date of birth or social security number:

1. Online/Fax

• go to the current students section from the MJC homepage, select "Quick References for Students", then College Forms.
• Scroll down to Address/Change of Information Form.  This is a pdf file.  You will need to print this form and follow the directions on it.

2. In Person

• You can come into the Records Office (Morris Memorial Rm 105) or the Admissions Office (East or West Campus) and pick up and complete the form.

It is important that the college have current contact information on every student and it is your responsibility to report any changes of your information. Failure to do so may prevent important information/notices to reach you, block you from viewing your own information, adding or dropping classes, etc.

PLEASE NOTE: When submitting this form (either in person or by mail or FAX) a legible copy of a valid photo ID must be provided with this request. Additional proof may be required. Incomplete requests WILL NOT be processed.
