INSTRUCTIONS:     Complete this form and return it to the Business Office.  See Schedule of Classes for Refund Policy.  Call 588-5114 for questions.  A check will be mailed to you within 6 - 8 weeks if eligible for the refund.  See Procedure for Refunds and deadline dates in the current semester Schedule of Classes.
   
NOTE:      THIS FORM DOES NOT WITHDRAW YOU FROM CLASSES.  Prior to the deadline, a student requesting a refund must officially drop classes at the Admissions & Records Office, on the college website, or through the Touchtone Telephone Registration System.
