Student Petition

Limitations:

1) Issues that relate to missed deadlines are not petitionable.

2) Graduation requirements are not petitionable.

Form Available: Office of Student Success, Library Basement, Room 20

Use this form to petition those issues to be submitted directly to the Office of Student Success for Petitions Committee review. Examples of petitionable items are

   (1) challenge of academic record and

   (2) withdrawals with verified extenuating circumstances.

Instructions: Form is completed by student. Depending on issue, may be forwarded to instructor, Division Dean for input.

Reviewed/Approval by: Petitions Committee. The Petitions Committee meets the first Wednesday of each month during the academic year. The deadline for submission is the preceding Friday. Applications submitted after this time will be reviewed the following month.

Student Notification: The Office of Student Success will send a letter to the student within 10 days of committee review.
